Refuah Shelemah (get well) prayer

When making a prayer for refuah shelemah - a full recovery to health -
for someone it is customary to name the sick person as the son or
daughter of their mother rather than father, eg David ben Sarah. What
if the sick person is a cohen or levi? Does the cohen or levi suffix to
the name attach to the individual (David) or to his father? Would one
say a refuah shelemah for "David ben Sarah *Hacohen*" if David himself
is a cohen? Are there different customs for this practice?
